A Review Finds Most Honey Clinical Trials Skipped Basic Quality Checks

A Slovak apitherapy researcher went looking through the honey clinical trial literature for one basic detail, what kind of honey people were actually given, and struggled to find it in most studies. Reviewing systematic reviews and meta analyses published between 2019 and 2023, the paper traces 28 individual clinical trials that tested honey against upper respiratory infections, cough, cholesterol, and blood sugar, and finds that only five of them used raw, unprocessed honey. Just one study measured hydroxymethylfurfural and diastase activity, the two standard markers of heat damage, and only two studies tested whether the honey actually had any measurable biological activity before giving it to patients.
The review worked by combing PubMed, Scopus, and Web of Science for recent meta analyses covering honey as a functional food, then pulling out the individual trials each one relied on. Most of those trials never stated the honey's botanical origin at all, listing it simply as natural honey. The author argues that this gap helps explain why honey trials so often contradict each other. Two recent meta analyses on lipid profiles reached opposite conclusions about whether honey lowers cholesterol, and the review suggests that unknown, uncharacterised honey is part of the reason.
The paper also flags weaknesses in the legal quality markers themselves. Diastase activity, tied to enzymes bees add during ripening, is naturally low in some varieties like citrus and manuka honey, and testing elsewhere found that nearly 37 percent of Turkish honey samples had foreign diastase added to mask this. Heat processing can quietly reduce a honey's antibacterial strength even while its HMF and diastase numbers stay within legal limits, since those two markers were never designed to track biological potency.
This is a review of existing literature, not a new laboratory experiment, and its conclusions rest on how thoroughly the underlying trials reported their methods rather than on fresh testing. It cannot say how much any single honey's real world effect changed with processing, only that almost nobody checked.
